Abstract

Epitheliale Tumore zeichnen sich histopathologisch durch ein substanzielles, desmoplastisches Tumorstroma aus, welches die Tumorzellinseln umgibt. Die Hauptzellpopulation dieses Tumorstromas bilden Krebs-assoziierte Fibroblasten (CAFs). Diese zeichnen sich durch eine permanente Aktivierung aus, welche mit der Expression eines CAF-assoziierten Phänotyps einhergeht. Inwieweit dieser Phänotyp in der Lage ist, spontane T-Zellantworten zu induzieren und ob CAFs damit als neue zusätzliche Quelle von T-Zellantigenen dienen können, sollte im Rahmen dieser Dissertation untersucht werden. Dafür wurde ein neuartiger Ansatz gewählt, der auf der Kombination der PF2D-ELISPOTMethode mit einem auf Microarraydaten basierenden Filterprozess beruhte. Zunächst wurden Tumorzellen und korrespondierende CAFs aus HNSCC-Tumorgeweben isoliert, charakterisiert und der Aktivierungsstatus der CAFs verifiziert. Mittels IFN-γ-ELISPOT-Assay konnten reaktive T-Zellen sowohl gegen Tumor- als auch gegen CAF-Lysate detektiert und damit bestätigt werden, dass auch CAFs immunogene Strukturen aufweisen. Mittels der PF2DELISPOT-Methode wurde das komplexe CAF-Proteom dann stufenweise reduziert und parallel eine Anreicherung der immunogenen Bestandteile bewirkt. Massenspektrometrisch wurden 852 Proteine als potenzielle Zielstrukturen für die T-Zellantworten identifiziert. Über Intersektionsstudien, die auf den Microarray-Expressionsdaten eines in situ- sowie eines in vitro-Datensatzes beruhten, wurden gezielt 36 CAF-spezifische Kandidaten herausgefiltert, deren immunogenes Potenzial dann über synthetische Peptide in einem IFN-γ-ELISPOT-Assay verifiziert wurde. Insgesamt konnten so 15 neue CAF-assoziierte T-Zellantigene identifiziert werden. Fünf dieser Peptide wurden ausgeschlossen, da sie auch in tumorfreien Individuen T-Zellantworten auslösten. Die Immunogenität der verbleibenden zehn Peptide wurde in einer Kohorte von 19 HNSCC-Tumorpatienten untersucht. Dabei zeigte das TXNDC17-Peptid mit einer Ansprechrate von fast 40 % die häufigste und zugleich die stärkste Immunantwort, gefolgt von DNAJB11 und NANS mit 21 % sowie von THBS2 und HIST2H2AC mit 16 %. Expressionsuntersuchungen ergaben für DNAJB11, NANS, TXNDC17 und THBS2 verglichen mit den Normalschleimhautproben erhöhte mRNA- sowie Proteinexpressionslevel im Tumor. Zudem konnte über immunhistochemische Färbungen eine Stroma-assoziierte Expression der Kandidaten nachgewiesen werden. Hingegen wurde HIST2H2AC in allen untersuchten Normalschleimhautproben nachgewiesen.

Insgesamt ist es uns gelungen, mehrere vielversprechende CAF-assoziierte Antigene zu identifizieren, die eine Tumorstroma-assoziierte Expression aufweisen (i), T-Zellen aus der Peripherie von Tumorpatienten aktivieren (ii), jedoch keine Reaktivität in gesunden Spendern hervorrufen (iii) und damit neben FAP immunogene Strukturen des Stromas darstellen (iv).Inwieweit sich diese neu identifizierten Antigene für immuntherapeutische Anwendungen in HNSCC eignen, müssen weiterführende präklinische Untersuchungen zeigen.

Translation of abstract (English)

Epithelial-derived tumors are histopathologically characterized by a desmoplastic stromal compartment which surrounds the malignant cancer cells. This tumor stroma mainly consists of cancer-associated fibroblasts (CAFs) which are characterized by an irreversibly activated phenotype that is presumed to be associated with the expression of CAF-associated antigens. Here, we focused on the identification of immunogenic CAF-associated T cell target antigens(CAF-TAs) using a novel approach which combines the PF2D-ELISPOT-method with a microarray data-based filter process. CAFs and corresponding tumor cells (TUCs) were isolated from freshly resected HNSCC tumor tissues using tissue explantation method following several in depth characterization steps including verification of the CAF activation status. We detected significant T cell (TC) responses against CAF and TUC lysates. Interestingly, the amount of CAF-reactive TCs was comparable to TUC-reactive TCs, indicating that CAF-TAs might also play a role in tumor immune responses. By applying the PF2D-ELISPOT-method CAF proteome was fractionated and in parallel enriched for immunogenic proteins. Subsequent mass spectrometric analysis of immunogenic fractions resulted in 852 potential immunogenic candidates. The intersection-based filter process resulted in 36 potential CAFassociated antigens. Immunogenicity of candidates was verified by the use of long synthetic peptides in a peptide ELISPOT. As a result, we have identified 15 novel CAF-associated target antigens of spontaneous TC responses. In a cohort of nine healthy individuals and 19 HNSCC patients we have shown that five (DNAJB11, NANS, HIST2H2AC, TXNDC17, THBS2) of this newly identified CAF-TAs triggered TC responses in 16 to 40 % of cancer patients, whereas no reactive TCs were detected in healthy individuals. Furthermore, expression analysis of two in situ data sets confirmed overexpression of these antigens in HNSCC tissues as compared to dysplasia-free oral mucosa samples. Immunohistochemical staining further corroborated a stroma-associated expression which was in case of THBS2 and DNAJB11 even limited to the stromal compartment. In contrast, epithelial cells and stromal cells of normal mucosa showed a strong HIST2H2AC expression. In conclusion, we have successfully shown that CAFs might serve as additional source of antigens,since we identified a set of novel CAF-TAs that are highly immunogenic and expressed in the stromal compartment of HNSCC tumors.
