Abstract

Background This thesis aims to record and analyse retrospectively the treatment of patients who suffer from an enterocutaneous fistula and who have been treated for it during the years 1995 till 2005 at the surgical department of the University of Erlangen. According to the path that was chosen to treat them – either surgically or conservatively (group A or B) – patients were divided into two groups. With the information gathered, the decisionmaking of the surgeon for one of the paths should be supported by reliable arguments. Methods In order to acquire the patients’ information, all files in the archives of the surgical department of the University of Erlangen led by the diagnosis of “enterocutaneous fistula” were analysed by a self-developed questionaire. Afterwards our results were compared with the findings of the current international specialist literature. Results Crohn’s disease and suture’s insufficiency were the main reasons for the genesis of ECF in both of the groups, but there were differences in the treatment: patients with Crohn’s disease were treated operatively more than twice as much than conservatively. In return, there were more patients suffering from cancer (often in advanced/palliative situations) in group B, which is also why the mortality rate was about twice as high as in group A. In both groups the site of the fistula was located mostly in the small bowel, the ileum as origin had a statistically significant impact on the treatment result. Sepsis as a complication also showed a statistically significant impact on the results and downgraded the prognosis of healing and survival. It is important to mention that patients of group B suffered from more severe intraabdominal inflammation than of group A. The duration of treatment was longer overall in group A than in group B, but patients who had to undergo intensive care treatment stayed there for longer when they belonged to group B. Our overall treatment showed the following treatment results: The fistula was healed completely in 69% of the patients (A 83%, B 34%). It did not heal completely or was healed on discharge in 22% (A 10%, B 52%). Nine percent of the patients died (A 7%, B 14%). A statistically significant relationship between the number of operations and the result of treatment could be found: the less operations, the better the treatment results, but a conservative treatment had the worst results with a healing rate of 34,5%. Compared to the current international specialist literature, our results are average. Conclusion As enterocutaneous fistulas are a rare problem, they should be treated at a centre that has experience with this difficult condition. As a matter of principle, the fistula should be operated on with tremendous care and the operation should be well prepared. Especially the group of patients risking to get an enterocutaneous fistula (i.e. patients with Crohn’s disease, operations on the small bowel), profit from a meticulously planned operation. In this process, the construction of a protective stoma (especially in secondary operations after persistent or recurrent fistula) is important to be on the safe side. Then, even patients who suffer from Crohn’s disease, do benefit.

Abstract

Hintergrund und Ziele Ziel dieser Arbeit ist die Erfassung und retrospektive Analyse der Behandlung von Patienten (=Pat.), die an einer enterokutanen Fistel (=ECF) erkrankt sind und im Zeitraum vom 1.1.1995 bis 31.12.2005 an der Chirurgischen Universitätsklinik Erlangen behandelt wurden. Dabei wurden die Pat. entsprechend des Behandlungspfades – Operation (= Gruppe A) oder konservatives Vorgehen (= Gruppe B) – in 2 Gruppen geteilt. Dadurch soll die für den Kliniker oft schwierige Entscheidungsfindung für einen der beiden Behandlungspfade erleichtert werden. Anschließend werden die Ergebnisse der Erlanger Untersuchung mit denen aus der aktuellen internationalen Fachliteratur verglichen. Methoden Zur Rekrutierung des Patientengutes wurden alle Patientenakten, die unter der Diagnose „enterokutane Fistel“ im o.g. Zeitraum im Archiv der Chirurgie der Universität Erlangen geführt wurden, herausgesucht und mittels eines eigens erarbeiteten Datenerhebungsbogens (siehe Anhang) schriftlich erfasst. Ergebnisse und Beobachtungen Unterschiede zwischen Gruppe A und B ergaben sich vor allem in der Ätiologie der Fisteln. Generell stellten Morbus Crohn (=MC) und Nahtinsuffizienz die häufigsten Ätiologien dar, wobei bei MC Patienten mehr als doppelt so häufig der operative Weg gewählt wurde. In Gruppe B lagen anteilig deutlich mehr Karzinom-assoziierte Fisteln vor (17% vs. 1% in Gruppe A). Die Mortalität in Gruppe B war circa doppelt so hoch wie in Gruppe A. In beiden Gruppen gingen die meisten Fisteln vom Dünndarm aus. Das Ileum als Ausgangspunkt der Fistel ist ein negativer Prädiktor für das Überleben (p=0,037). Die Sepsis als Komplikation zeigte einen signifikanten Einfluss auf das Behandlungsergebnis (p=0,002) und verschlechterte die Prognose der Heilung und des Überlebens, wobei es bei Pat. der Gruppe B generell zu schwereren intraabdominellen Entzündungen kam. Die Behandlungsdauer war in Gruppe A insgesamt länger als in Gruppe B, jedoch war die Behandlungsdauer auf Intensivstation für Patienten der Gruppe B länger als für Patienten der Gruppe A. Das Behandlungsergebnis gestaltete sich folgenderweise: Die Fistel war bei insgesamt 69% der Pat. (A 83%, B 34%) geheilt. Sie blieb bei insgesamt 22% bestehen oder heilte nicht komplett (A 10%, B 52%) und 9% der Pat. starben während ihres stationären Aufenthalts (A 7%, B 14%). Wir konnten einen signifikanten Zusammenhang der Anzahl der Operationen mit dem Behandlungsergebnis feststellen (p=0,001): je weniger Operationen, umso besser das Behandlungsergebnis, aber eine konservative Behandlung hatte die schlechteste Heilungsrate mit 34,5 %. Im Vergleich mit der aktuellen internationalen Fachliteratur liegen unsere Ergebnisse im Durchschnitt. Praktische Schlußfolgerungen Aufgrund der Seltenheit des Auftretens von enterokutanen Fisteln sollten sie in einem Zentrum behandelt werden, wo man Expertise mit diesem schwierigen Krankheitsbild hat. Besonders bei Risiko-Patientengruppen mit einem Morbus Crohn als Grunderkrankung ist eine sorgfältig geplante und auf Sicherheit ausgelegte Operation wichtig. Dabei spielt die Anlage eines protektiven Stomas (vor allem bei Zweitoperationen nach Fistelpersistenz oder -widerauftreten) eine wichtige Rolle. Dann profitieren Patienten, die an MC erkrankt sind, in der Regel von der Operation.
