Funktionelle und radiologische Ergebnisse nach Arthrodesen der Sprunggelenke

Abstract

In den Jahren 1994 bis 1999 behandelten wir 47 Patienten mit insgesamt 48 Arthrodesen im Bereich des oberen und/oder unteren Sprunggelenks. 41 Patienten mit 41 Arthrodesen konnten im Durchschnitt 29 (6-65) Monate nach erfolgter Arthrodese klinisch und radiologisch nachuntersucht werden. Als Hauptindikation führte in unserem Patientenkollektiv die symptomatische posttraumatische Arthrose zur Gelenkversteifung. Hinsichtlich der Arthrodesetechnik überwog die Kompressionsschraubenarthrodese bei weitem andere mögliche Methoden. Komplikationen beobachteten wir in 33 % der Fälle, wobei neben 4 tiefen Infektionen und 5 Arthrodeseninsuffizienzen blande postoperative Wundheilungsstörungen im Vordergrund standen. Die klinisch und radiologisch erhobenen Untersuchungsergebnisse wurden nach 3 Score-Systemen ausgewertet. Nach dem repräsentativen und international gebräuchlichen Score von KITAOKA konnten wir in unserem nachuntersuchten Patientenkollektiv insgesamt 68 % sehr gute und gute, 22 % befriedigende und 10 % schlechte Ergebnisse verzeichnen.

Between 1994 and 1999 we treated 47 patients with 48 arthrodesis in the area of the ankle joint and/or the the subtalar joint. 41 patients with 41 arthrodesis were examinated physical and radiographical for an average of 29 months (6-62 months) after the arthrodesis operation. As meanindication for arthrodesis in our patientgroup we concluded the symtomatic post-traumatic arthrosis. In reference to the arthrodesis technique the compression arthrodesis with scew fixation predominated other possibilities. Complications were found in 33% of the cases, superficial postoperative wound problems predominating 4 deep infections and 5 nonunions of arthrodesis. The clinical and radiographic results where evaluated following 3 score systems. By using the representative and international accepted score of KITAOKA we specified 68% very good and good, 22% satisfactory and 10% poor results.
