Abstract: Der Gegenstand der Arbeit ist die Modellierung des kardialen Transports, der Bindungskinetik und des inotropen Effektes von Amiodaron, Verapamil und α-adrenerger Pharmaka. Die Modellierung diente dazu, pharmakokinetische und pharmakodynamische Daten auszuwerten, die am Langendorff-perfundierten Rattenherz gemessen wurden. Amiodaron zeigte eine schnelle Aufnahme ins Herzgewebe, einen hohen Partitionskoeffizienten und eine lange Gewebeauswaschzeit von 17 h. Der negative inotrope Effekt von Amiodaron entwickelte sich verzögert im Vergleich zu den Konzentrationen im Gefäßkompartment und konnte gut mit dem Maximum-Effekt-Modell mit Emax = 37.7% und EC50 = 16.4 nM beschrieben werden. Der negative inotrope Effekt von Verapamil wurde ebenfalls durch ein sigmoidales Emax-Model mit Emax = 50.5 mmHg und EC50 = 16.4 nM beschrieben; auf Grund des Signaltransduktionsprozesses war der Zeitverlauf des Effektes im Vergleich zur Gewebekonzentration verzögert. Das pharmakodynamische Model des inotropen Effektes von Verapamil erklärte auch den positiv-inotropen Rebound-Effekt. Eine P-Glykoprotein-Hemmung durch Amiodaron hatte keinen Effekt auf die kardiale Aufnahme von Verapamil, während eine Endotoxemie diese signifikant verminderte. Eine Endotoxemie erzeugte auch eine signifikante Verminderung von Emax. Ein mechanistisches Modell wurde entwickelt, um die Wechselwirkung zwischen dem α1-adrenergen Agonisten Phenylephrin und dem selektiven α1-adrenergen Blocker Prazosin zu beschreiben und die kardiale α1-Rezeptor-Konzentration, die Assoziations- und Dissoziationskonstanten der Rezeptor-Liganden gleichzeitig mit der Stimulus-Response-Beziehung zu bestimmen. Unter Anwendung des "Operational models" für Rezeptoragonisten im dynamischen Zustand war es möglich, zwischen Rezeptorbindung und Signaltrans-duktionsprozess als Determinanten des inotropen Effektes für Phenylephrin zu unterscheiden. Für Phenylephrin und Prazosin wurden die Gleichgewichtsdissoziationskonstanten von 98.6 bzw. 0.08 nM geschätzt; für α1-Rezeptor Konzentration im linken Ventrikelgewebe des Rattenherzens lieferte das Model einen Wert von 50 pmol/g.
The objective of this work was the modelling of cardiac uptake, binding kinetics and inotropic response of amiodarone, verapamil and α1-adrenergic agents in order to analyze pharmacokinetic and pharmacodynamic data measured in Langendorff perfused rat hearts. Amiodarone was characterized by a rapid uptake into the heart, a high partition coefficient and a long tissue washout half-life of 17 h. The negative inotropic effect of amiodarone was delayed with respect to the concentrations in vascular compartment and well described by the maximum effect (Emax) model with Emax = 37.7% and EC50 = 0.53 mμ M. The negative inotropic effect of verapamil was also described by a sigmoid Emax model with Emax = 50.5 mmHg and EC50 = 16.4 nM and its time course was linked to tissue concentration with a delay due to the signal transduction process. The pharmacodynamic model of the inotropic effect of verapamil also accounted for the positive inotropic rebound effect. P-glycoprotein inhibition by amiodarone had no effect on cardiac uptake of verapamil, whereas endotoxemia significantly decreased it. Endotoxemia also caused significant reduction of Emax. A mechanism-based model was developed to study the interaction of the α1-adrenergic agonist phenylephrine with the selective α1-adrenergic blocker prazosin in order to estimate cardiac α1-receptor concentration, the association and dissociation constants of receptor ligands simultaneously with the stimulus-response relationship. Using this operational model of receptor agonists in the transient case, it was possible to differentiate between receptor binding and of signal transduction process as determinants of the inotropic effect of phenylephrine. Equilibrium dissociation constants of 98.6 and 0.08 nM, were estimated for phenylephrine and prazosin, respectively; the model estimate of the α1-receptor concentration in the left ventricular tissue of the rat heart was 50 pmol/g.
