Problemstellung: In dieser Studie sollte die Genauigkeit der mesiodistalen Zahnwurzelangulationen aus 3D DVT-Datensätzen, sowie rekonstruierten- und konventionellen digitalen PSA gegenübergestellt und hinsichtlich der Einflussfaktoren bukkolinguale Angulation, Stärke der Zahnbogenkrümmung und Positionierungsfehler untersucht werden. Zudem wurde die Erfassung von bukkolingualen Zahnwurzelangulationen aus 3D DVT Datensätzen geprüft. Material und Methode: Zunächst wurden zwei Modelle mit insgesamt 56 Zylinderbohrungen mit festgelegten Angulationen in mesiodistaler und bukkolingualer Richtung angefertigt und nach röntgenopaker Markierung mittels des dentalen Volumentomographen KaVo 3D eXam und des konventionellen digitalen Orthopantomographen Orthophos DS Ceph Plus radiologisch untersucht. Die Rekonstruktion der Panoramadarstellung aus den 3D DVT Datensätzen und die Ermittlung der Angulationen der DVT Aufnahmen erfolgte mittels der 3D Imaging Software Invivo 5 bzw. der Analyse opgsie der Diagnostiksoftware FR-Win für die mesiodistalen Angulationen der rekonstruierten und konventionell digitalen PSA. Weiterhin wurden vorhandene Dentale Volumentomographien und konventionelle Panoramaschichtaufnahmen von drei Patienten analog der Modelle vermessen und unter Berücksichtigung der Erkenntnisse der Modelluntersuchungen interpretiert. Ergebnisse: Die beiden Methoden der dreidimensionalen Röntgentechnik aus den direkten 3D Messungen und den rekonstruierten Panoramadarstellungen liefern hochgenaue Ergebnisse hinsichtlich der mesiodistalen, sowie der bukkolingualen Angulationen der Zylinder für die untersuchten Modelle. Die Darstellungen der konventionellen Panoramaschichtaufnahmen weisen erhöhte Abweichungen der ermittelten mesiodistalen Zylinderneigungen in Abhängigkeit der Höhe der bukkolingualen Winkel, sowie der Modellpositionen während der Röntgenaufnahmen auf. Der Einfluss der Zylinderbogenkrümmung konnte in dieser Studie aufgrund des komplexen Versuchsaufbaus nicht hinreichend geklärt werden. Die Ergebnisse der Patienten zeigen, analog zu den Modellvermessungen, eine hohe Übereinstimmung der rekonstruierten PSA mit denen der direkten Messungen der 3D DVT Datensätze (Goldstandard). Die Abweichungen des konventionellen Verfahrens sind hingegen deutlich höher. Die Begründungen der Darstellungsungenauigkeiten im Rahmen der Betrachtungen der o.g. Einflussfaktoren lassen sich auf die Messungen der Patienten gut übertragen. Schlussfolgerungen: Die Analyse der mesiodistalen Zylinderangulationen aus 3D DVT Datensätzen und den daraus rekonstruierten Panoramadarstellungen sind der konventionellen Panoramaschichtaufnahme deutlich überlegen. Die direkte Bestimmung der bukkolingualen Zahnwurzelangulationen aus den 3D DVT Datensätzen bietet einen zuverlässigen Mehrgewinn im Rahmen der kieferorthopädischen Diagnostik.
Introduction: The purpose of this academic research is to define the accuracy of mesiodistal angulation of the tooth root from 3D DVT datasets and reconstructed, as well as conventional digital PSA, confronted and being evaluated on the following influencing factors: buccolingual angulation, amount of jaw curvature and positioning errors. Furthermore, 3D DVT datasets have been evaluated to identify if the buccolingual angulation of the tooth roots have been captured in the data. Materials and methods: In the first instance, two models with a total of 56 cylindrical drillings and set angulations in mesiodistal and buccolingual direction were custom built. After the application of a radiopaque marker these models have undergone a radiological examination using both, the digital volume tomography unit (CBCT) KaVo 3D eXam, as well as a conventional digital panoramic radiograph Orthophos DS Ceph Plus. The reconstruction of the panorama image from the 3D DVD datasets and the determination of the angulations of the DVT images was done using the 3D imaging software Invivo 5, as well as by analyzing the diagnostic software FR Win for the mesiodistal angulations of reconstructed and conventional digital PSA. Furthermore, existing dental volume tomography data and conventional panorama tomography images of three patients were measured and, with simultaneous consideration of the results of the model examinations, interpreted. Results: Both methods of the three-dimensional radiological examination, from the direct 3D measurement and the reconstructed panorama display, deliver high-precision results with regards to the mesiodistal as well as the buccolingual angulations of the cylinder of the models examined. The representation of the conventional panorama tomography show considerable deviations of the measured mesiodistal cylinder angle, depending on the level of the buccolingual angle and on the position of the model at the time of taking the X-ray image. Due to the complex nature of the experimental set up, the effect of the cylinder curvature could not sufficiently be explained. The results of the patients show, in keeping with data from model measurements, a high accordance of the reconstructed PSA with the direct measurements of the 3D DVT data sets (gold standards). The justifications of the imprecisions of the presentations in the course of the observations of the mentioned influencing factors can well be transferred on the measurements of the patients. Conclusions: The analysis of the mesiodistal cylinder angulation from 3D DVD datasets and the resulting panorama images is far superior to the conventional panorama tomography. The direct determination of the buccolingual angulations of the tooth root from 3D DVT datasets offers a reliable added value within the scope of orthodontic diagnostic.
