Einleitung: Das Rauchen von Tabak ist weltweit die häufigste vermeidbare Todesursache und bedingt aktuell jährlich etwa 5 Millionen Todesfälle mit weiterhin steigender Tendenz. Nikotin ist der hauptsächliche psychoaktive Bestandteil des Tabakrauchs und entfaltet seine Wirkung u.a. über die Ausschüttung von Dopamin im limbischen System. Von besonderer Bedeutung ist hierbei der nikotinische Acetylcholinrezeptor, über den eine vermehrte Dopaminausschüttung im Nucleus accumbens nach Nikotinzufuhr, mit daraus resultierendem Belohnungsgefühl, ausgelöst wird. In mehreren Arbeiten konnte gezeigt werden, dass die Nikotinabhängigkeit durch genetische Determinanten beeinflusst wird und dass genetische Varianten des COMT-Gen, genauer gesagt der Val158Met-Polymorphismus (SNP rs 4680) Einfluss auf die Nikotinabhängigkeit haben kann. Das ereigniskorrelierte Potential P300 ist ein Marker für Arbeits- und Aufmerksamkeitsprozesse. Es konnte eine Amplitudenreduktion bei Rauchern sowie bei Patienten mit anderen psychiatrischen Erkrankungen im Vergleich zu Gesunden nachgewiesen werden. Die P300 unterliegt ebenfalls genetischen Determinanten. In der vorliegenden Arbeit wird zum einen die bereits mehrfach gezeigte Amplitudenreduktion der P300 bei Rauchern gegenüber Nichtrauchern repliziert sowie ihre mögliche Abhängigkeit von genetischen Varianten des SNP rs4680 des COMT-Gens untersucht. Methodik: Bei 378 gesunden Probanden, davon 171 Nichtraucher und 207 Raucher, wurde die akustisch evozierte P300-Komponte frontal gemessen sowie Blutproben zur Untersuchung der Genotypverteilung des SNP rs4680 gewonnen und ausgewertet. Ergebnisse: Es konnte eine signifikante Amplitudenreduktion der P300 an der frontalen Ableitung bei Rauchern gegenüber Nichtrauchern in Abhängigkeit von den Kofaktoren Geschlecht und Alter gezeigt werden (p=0,007). Weiterhin zeigte sich in der Gruppe der Raucher keine weitere Amplitudenreduktion an der Fz-Elektrode mit steigender Konsummenge (Zigaretten/d) oder steigenden Packungsjahre. Der Einfluss des Genotyps des SNP rs4680 im COMT-Gen auf den Raucherstatus zeigte sich als nicht signifikant (p=0,516). Weiterhin konnte auch kein signifikanter Einfluss des Genotyps des SNP rs4680 auf die P300-Amplitude nachgewiesen werden (p=0,088) - auch nicht in Interaktion mit dem Raucherstatus (p=0,102). Schlussfolgerung: Das Ergebnisse dieser Arbeit konnte den signifikanten Unterschied der P300-Amplitude zwischen Rauchern und Nie-Rauchern replizieren, eine Korrelation zwischen dem Grad des Nikotinkonsums und dem Ausmaß der Amplitudenreduktion der P300 konnte jedoch nicht repliziert werden. Ein Einfluss des Genotyps SNP rs4680 im COMT-Gen lässt sich weder bezüglich des Raucherstatus noch bezüglich der Amplitudenreduktion der P300 bei Rauchern beobachten. Es sind sicherlich weitere Untersuchungen notwendig, um festzustellen, ob sich eine genetische Ursache für diese Effekte finden lässt und welche Gene bzw. Varianten diese beeinflussen.
Introduction: Smoking is still the biggest single form of avoidable death worldwide. There are 5 million deaths due to smoking every year, with this figure set to increase in future. Nicotine is the main psychoactive component of tobacco smoke and spreads its effects through the release of dopamine in the limbic system. Inter alia this takes place in the frontal lobe. The nicotinic acetylcholine receptor is of particular importance, because it stimulates an increased dopamine secretion in the nucleus accumbens after nicotine intake. This in turn leads to a feeling of reward. Several studies have shown, that nicotine addiction is influenced by genetic determinants. Genetic variants in the COMT-gene, more precisely the Val158met polymorphism (SNP rs4680) have an impact on nicotinic addiction. The event-related potential P300 is a marker for work and attention processes. An amplitude reduction could be proved among smokers and patients with other psychiatric disorders compared to healthy persons. The P300 is also influencedby genetic determinants. This paper aims to replicate the amplitude reduction of the P300 amongst smokers compared to non-smokers and to examine its possible dependence on genetic variants of SNP rs4680 in the COMT-gene. Methodology: 378 healthy test persons, 171 non-smokers, 207 smokers participated in the study. The auditory evoked P300 component was frontally measured in all test persons. Furthermore, genetic material was acquired and analysed to examine the genotype distribution of SNP rs4680. Results: The study could show that smokers have a significant amplitude reduction of the P300 on the frontal recording compared to non-smokers. For this result the cofactors age and sex (p= 0,0007) were taken into account. The collective of the smokers didn’t show an increased amplitude reduction at the fz-electrode when bigger quantities were consumed (cigarettes/d) or the test persons had a higher amount of pack years. The influence of the genotype SNP rs4680 in the COMT- gene on the smoking status was insignificant (p=0,5616). Furthermore, the genotype SNP rs4680 didn’t have a significant influence on the P 300 amplitude (p=0,088) - not either in conjunction with the smoking status. Conclusion: The results of this paper indicate, that there is a correlation between nicotine consumption and the reduction of the P300 amplitude. Other studies confirm this result. An influence of the genotype SNP rs4680 in the COMT-Gen on the smoking status or the P300 amplitude reduction couldn`t be observed. Further studies will be necessary to find out, if there is a genetic cause for this effects and which genes or variants are due to have an influence.
