Abstract: Die diagnostische Wertigkeit von Computertomographie und Sonographie zur Qualitätskontrolle bzw. -sicherung von Operationsergebnissen am Larynx wurde überprüft. Bei 45 transsexuellen Patientinnen, die sich einer Kehlkopfoperation zur Stimmerhöhung unterzogen, wurden hierzu prä- und postoperativ mittels Computertomographie (Spiral-CT) und Sonographie der cricothyroidale Abstand und die Stimmbandlänge bestimmt. Ein Vergleich beider diagnostischer Verfahren zur Bestimmung der diagnostischen Wertigkeit, Genauigkeit und zur postoperativen Kontrolle ergab eine gute präoperative Übereinstimmung beider Verfahren, postoperativ fanden sich jedoch Abweichungen. Eine Korrelation der Stimmerhöhung mit dem cricothyroidalen Abstand lag nicht vor. Lediglich die Differenz der Stimmbandlänge prä- und postoperativ korrelierte mit der Stimmerhöhung in Halbtonschritten postoperativ. Die Strahlenexposition der Patientinnen im CT wurde berechnet. Zusammenfassend ergab sich, dass die CT-Untersuchung prä- und postoperativ nur einzelnen Problemfällen vorbehalten bleiben sollte, nicht zuletzt auf Grund der Strahlenexposition für die Patientinnen.
Cricothyroid approximation raises the vocal pitch by simulating the contraction of the cricothyroid muscle with sutures. The aim of this study was to determine the role of spiral computed tomography (CT) in transsexual patients scheduled for cricothyroid approximation. 45 transsexual patients were examined with spiral CT and ultrasound before and after laryngoplastic surgery. CT and ultrasound accurately determines the cricothyroid distance before surgery, after surgery was found a difference between both methods. The vocal pitch elevation did not correlate with the reduction of the cricothyroid distance. Though the difference of the vocal cord before and after surgery correlated with the vocal pitch elevation after surgery. The x-ray exposition was calculated. The conclusion is that CT should be used in some problematic cases only, not at least to decrease the x-ray exposition.
