Four holes are burned in last folia, from end of codex back through f. 134 (from acid?, leaving red-brown stains around edges of holes), and lower corners also excised from ff. 135-139. Many folia badly worn, esp. at front and end of codex.

[1], i2-1 (one removed at front; perhaps originally together with next quire), ii6 (perhaps originally with f. 1, making a quire of 8-1, lacking 8th, possibly an illustration), iii-vi8, vii8-1, (lacks original 4th [after 42], would have contained last 12 verses of Matthew on recto and portrait of Mark on verso), viii-xviii8, xix6-1 (lacks one at end, no text lost), [1]. No quire signatures.

One main hand (same hand in smaller module for prefaces), plus at least three minor hands for quires xviii-xix (ff. 127ra-138vb, last few verses of John plus Capitulary).

- Full-page paintings of Matthew, Luke, and John ff. 10v, 64v, 101v (seated, writing, elaborate framing, in browns and tans, white, pink, blue, yellow, and red; derived from Greek or late antique models);
- canon tables framed in painted column-and-cornice designs, with full-page titulus in similar frame, in large square capitals, alternate lines red and black;
- initials in 2- to 4-line red square capitals;
- verse initials in square or rustic capitals;
- tituli in 1- to 4-line square and/or rustic capitals (hierarchy);
- first lines for Matthew in uncials, for Mark in rustic capitals, for Luke and John square and rustic capitals (hierarchy);
- first lines of Gospel chapters in uncials, square or rustic capitals;
- initials, tituli, and first lines red or black.
- Additions in dry point: drawing of bust of saint f. 10r, lower margin;
- decoration in lower margin 31v.

Previously ascribed to the scriptorium of Freising (see Bischoff and Mütherich, opp. citt.). Place of origin given by Schulten, op. cit., as "Köln (?)".
